Elwyn seeks a Registered Nurse with experience in Adult Community Clinical Services (ACCS) and Respite Adult Behavioral Health settings. Provides comprehensive nursing care to adults with complex mental health conditions, co-occurring disorders, and substance use disorders. Experienced in delivering both generalized and specialized medical and behavioral health care, including assessment, crisis stabilization, medication management, and coordination of care.
Functions effectively as a member of a multidisciplinary rapid-response team, utilizing sound clinical judgment and independent nursing decision-making to support recovery-oriented, trauma-informed care. Maintains accurate documentation and ensures compliance with regulatory and organizational standards. DUTIES AND RESPONSIBILITIES
Provide on-site or mobile support, screening, Assessment, and Individual Action Planning
Ensure that the PRISM model is integrated into everyday practice and clinical interventions are being provided with each consumer
Incorporates evidence-based practice, group work, and wellness tools
Obtain/conduct and accurately document a relevant comprehensive health history, with an emphasis on mental health history
Assist individuals served in the management of mental and physical illness. Provides educational services designed to support and train consumers to self-administer and monitor their own medication in accordance with their IAP.
Provide staff and consumers with support in developing consumer's self-advocacy skills regarding the effective use of medications.
Administers specified medication and take vital signs as indicated and document accordingly
Reviews person's served records to ensure that medical goals are being addressed and to note recommendations for change and new clinical approaches as needed
Reviews referral packets and assess referrals for medical issues, supports linkages to community healthcare providers
Participates in individual action planning by utilizing obtained nursing assessment information, diagnostic results, lab work, testing, etc.
Participates in team meetings
Oversight of Medication Administration Program (MAP)
Conducts staff development in-services i.e., individual age/disability specific information, infection control, OSHA, organization policy and medical procedures, risk management, MAP policy and individual staff/program development.
Follows all current policies and procedures of Elwyn Massachusetts
Cooperates with Elwyn Massachusetts Department of Mental Health, or any other authorized investigating body during an inspection, an inquiry, or investigation.
Maintains compliance of the client records within the scope of nursing duties
Perform cardiopulmonary resuscitation (CPR), crisis intervention and medication administration using agency-trained protocols
Maintains current state-mandated training and certifications
All other duties as assigned
EDUCATION/SKILLS/EXPERIENCE REQUIREMENTS:
Nursing diploma, associate or bachelor's degree required
Valid Massachusetts Registered Nursing (RN) license required
Must be in good standing with the Massachusetts Board of Nursing
Experience with Electronic Health Record (EHR) / Electronic Medical Record (EMR) required
Demonstrated ability to work effectively as part of a team
Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
Knowledge of theory and principles of psychiatric nursing preferred
Knowledge of psychopharmacology, symptom management, observation of effects of medication, reporting and re
